Note 1: Requests must contain the following:
- Name, address, and phone number of requestor
- Detailed description of records being requests
- A willingness to pay fees associated with processing the request

Note 2: Commercial requesters should indicate a willingness to pay all search, review and duplication cost. Educational or news media requesters (i.e., universities, newspapers, TV stations) should indicate a willingness to pay assessable duplication charges in excess of 100 pages. All other requesters should indicate a willingness to pay assessable search and duplications costs if more than two hours of search effort and more than 100 pages are desired. Charges for contract documents will be based on previous requests for the documents. Fees are reduced accordingly for subsequent requests for contract documents. 

Note 3: If a special requirement exists to obtain requested records in a specific format (i.e., MS Word, Excel, Adobe PDF or other application), the requirement must be clearly stated in the request. FOIA requests not specifying a specific format will result in any responsive records being provided as an Adobe PDF read only electronic attachment to the final e-mail response, or provided on a DVD/CD-ROM disk sent via first class mail. Paper versions of responsive records will be provided in extraordinary cases where access to private or public computer resources cannot be realized.
