The Ehrling Bergquist Medical Group staff would like to thank the beneficiaries for their generous support of the Third Party Program. According to Public Law 101-510 (10 U.S. Code 1095), DOD Instruction 6010.15M, 4-2, and AFI 41-120, all non-active duty beneficiaries using any military medical facility must complete a DD Form 2569 (Third Party Collection Form) annually or whenever health information changes.

Besides meeting the legal requirements, providing information about third party insurance helps in two additional ways. First and foremost, charges billed to the member insurance companies helps satisfy annual deductibles with zero out of pocket cost to the member. Secondly, 100 percent of funds collected by the Ehrling Bergquist Clinic, $2.1 M in Fiscal Year 2008, is being reinvested in the medical facility. These funds enable the 55th MDG to provide the best medical equipment, supplies, renovations, and additional staff to meet healthcare needs.

Please present the DD Form 2569 to clinic personnel at each appointment or when picking up pharmaceuticals. Doing this will streamline the check in process and greatly reduce wait times.
