As the weather warms up and Team Offutt heads outside for morale events like ruck marches, 5Ks, burger burns and b
oating at the lake — it's the perfect time to build camaraderie and enjoy the season. But with all activities, no matter how routine they may seem, comes risk.
To ensure everyone stays safe, the Deliberate Risk Assessment Worksheet (AF Form 4437) must be completed for any event that takes place during duty hours and occurs outside the normal workplace environment. This includes activities that fall under “alternative duty locations.”
Each hazard should be identified and addressed on AF Form 4437. This form must be:
- Completed with support from your Unit Safety Representative (USR) or supervisor.
- Signed by your commander.
- Routed through the 55th Wing Occupational Safety Office for review before final approval.
Note: For unit-sponsored events open to all of Team Offutt, risk acceptance may be required from the Installation Commander, so early coordination with the Safety Office is essential.
